Screening is non-negotiable and inability to be screened by my preferred methods will result in no session. Please provide proof of your vaccination and one (or more) of the following:

 

-Two or more references from verifiable providers you have seen in the last year. Please include their name, email and social media/website

-A photo of you holding your ID, along with a photo of the ID itself and workplace verification (a LinkedIn profile, your name/photo/email listed on a company website, a blank email from your work email sent to a clean unaffiliated account). All photos/documents should be sent to my email in addition to this form.
